Supplemental Grade Report Form (SGRF)

The Supplemental Grade Report Form (SGRF) can be used to:

  • Change a graduate student’s “DFR” grade in a Thesis 499/599 course. The successful or unsuccessful completion of a Thesis 499/599 graduate course requires a SGRF to be submitted to change a student’s grade. For more detailed information on the Thesis 499/599 SGRF process, please refer to the Thesis Grades page.
  • Correct an ABS grade for a student that was absent from the final examination without an acceptable excuse. This process requires the signatures/approval from the following individuals: Course Instructor, Executive Office of the Department that offered the course, and the Dean of the student’s college at the time the original grade was assigned.

Undergraduate Procedure

Instructors

  • Contact your department for an SGRF.
  • Complete and sign the form.
  • Submit the SGRF to the department offering the course.

Department Offering the Course

  • Review the SGRF.
    • Verify the accuracy of the data.
    • Review signatures.
  • Retain a copy for your records.
  • Send the form to the student’s college office.
  • Review Academic History in Banner to verify that the form has been processed.

College Office

  • Review the form.
    • Verify the accuracy of the data.
    • Review signatures.
  • Retain a copy for your records.
  • Send the white copy to Academic Records; 901 West Illinois Street, Suite 140; Urbana, MC-061.

Graduate Procedure

  • Review the form.
    • Verify the accuracy of the data.

  • Review Academic History in Banner to verify that the form has been processed.

Important Reminders

Throughout this process, remember to exercise caution about the content of email messages. To protect the personal privacy and confidentiality of student information, do not mail grades out over insecure email. Unencrypted email is not a secure means of transmitting information.

  • Students can view grade corrections in UI-Integrate Self-Service once the Office of the Registrar has updated Banner.
  • Use the Academic History SHACRSE—Course Summary form to view official grades.
